Life journal to express end-of-life wishes

This document allows for the collection of personal wishes regarding end-of-life care, family, spirituality, and funeral arrangements, in order to ensure support that is respectful and in line with the person's values.

Part I: Close Persons and Healthcare Professionals

This section lists significant persons and the primary physician, with their contact information, to ensure communication and consideration of wishes.

  • Includes up to 3 children or stepfamily members, with the option to duplicate the record if more.
  • Mention the other close people or friends to contact.
  • Details of current and previous primary care physicians, including the start year of their care.

Volume II: Elements of Personal Experience

This section allows you to share life experiences, values, passions, and projects so that your loved ones can understand you better.

  • Includes highlights, main roles, achievements, current values, hobbies, passions.
  • Address the concept of dignity and what is important to the person.
  • Allows indicating projects to be completed or closed before end of life.

Part III: Quality of Life and End-of-Life Design

This section invites reflection on the definition of quality of life, dignity, and the ideal place to spend one's final moments.

  • Clarify the will regarding the acceptance of limitations to extend life.
  • Ask the question of whether or not to accept serious limitations in order to continue living.

Part IV: Spiritual and Philosophical Affinities

This section concerns spiritual or philosophical accompaniment, with the option to choose a counselor according to religious denomination or secularism.

  • Would you like to speak with a spiritual counselor?.
  • Please specify the type of advisor desired (lay, Catholic, Muslim, etc.).
  • Invitation to formulate specific wishes for spiritual accompaniment.

Part V: Post-Mortem Wishes

This section details wishes regarding the body, funeral, and scattering or preservation of ashes.

  • Include desired rituals, burial or cremation, cemetery location.
  • Allows to specify whether the person wishes to leave the choice to their loved ones or to take out funeral insurance.
  • Provide information about the insurance company and the contract.

Section VI: Sharing Words and Messages with Loved Ones

This section offers the option to express messages or ideas to convey to loved ones after death.

  • Allows a person to be designated to convey these words.
  • Offers a keepsake for posterity or for the family, enclosed in an envelope if desired.
